Hepatitis B Core Antibodies

The presence of these antibodies can indicate either a past or ongoing HBV infection. Anti-HBc antibodies typically appear shortly after the onset of infection or symptoms and can remain in the bloodstream for life, even after the infection has resolved.

There are two types of Anti-HBc antibodies:

IgM: The first antibody to appear after infection, typically indicating a recent or acute infection.
IgG: This antibody replaces IgM over time and is usually associated with a past infection or a chronic (ongoing) infection.

Anti-HBc antibodies are an important biomarker for diagnosing and understanding the course of a hepatitis B infection. However, they cannot distinguish between an active or past infection on their own. Additional tests, such as the Hepatitis B surface antigen (HBsAg) test, are required to get a complete understanding of a person’s hepatitis B status.

Hepatitis B surface antigen (HBsAg)
Hepatitis B surface antigen (HBsAg) is a protein found on the surface of the Hepatitis B virus. It is present in the blood of individuals infected with the virus.

When a person contracts Hepatitis B, HBsAg is the first marker to appear in the blood, often before symptoms develop. As such, a test that detects HBsAg is commonly used to diagnose an active Hepatitis B infection.

If HBsAg remains detectable for more than six months, it typically indicates a chronic Hepatitis B infection. If the antigen becomes undetectable within six months, it suggests that the person’s immune system has successfully cleared the acute infection.

Importantly, the presence of HBsAg in the blood also means the individual is infectious and can transmit the virus to others. This is particularly relevant in situations such as blood donation, needle sharing, sexual contact, and mother-to-child transmission.

While a positive HBsAg test confirms an active infection, additional testing is often required to determine whether the infection is acute or chronic and to help guide treatment decisions.

Early detection of HBsAg is crucial for managing Hepatitis B infections, as timely intervention can prevent long-term complications such as liver cirrhosis and liver cancer.

Human Immunodeficiency Virus
HIV (Human Immunodeficiency Virus) is a virus that attacks the body’s immune cells, making it more vulnerable to infections and diseases. If left untreated, HIV can progress to AIDS (Acquired Immunodeficiency Syndrome), a condition where the immune system is severely weakened.

In biomarker testing for HIV, the two most common biomarkers are:

HIV Antibodies: These are proteins produced by the immune system in response to HIV infection. The presence of HIV antibodies usually indicates that a person has been infected with the virus.

HIV Antigens: These are proteins produced by the HIV virus itself. The p24 antigen is the most commonly tested and can be detected in the blood within 2 to 4 weeks after HIV infection.

Most modern HIV tests use a combination of both antibodies and antigens, often referred to as a “4th generation” test. This method allows for earlier and more accurate detection of HIV compared to testing for antibodies alone. A positive result typically requires a follow-up test to confirm the diagnosis.

Here’s a basic outline of how these tests work:

A blood sample is collected.
The sample is tested for the presence of HIV antibodies and/or antigens.
If either HIV antibodies or antigens are detected, the result is considered positive, suggesting HIV infection.
A positive result is usually confirmed with a follow-up test, such as an HIV RNA test, which detects the virus’s genetic material.
It’s important to note that there can be a delay between the time of infection and when the antibodies and antigens are detectable. This is called the “window period,” which varies between individuals but is typically around three weeks.

Hepatitis B surface antibodies (Anti-HBs)

Hepatitis B surface antibodies (Anti-HBs) are produced by the immune system in response to the surface antigen of the Hepatitis B virus (HBsAg). These antibodies are a key biomarker for determining an individual’s immune status to Hepatitis B.

When detected in a home blood test, Anti-HBs generally indicate one of the following:

Recovery and Immunity: If Anti-HBs are present and the individual tests negative for Hepatitis B surface antigen (HBsAg), it typically means the person has recovered from a past Hepatitis B infection and is now immune. The immune system has successfully fought off the infection, and the body retains a “memory” of it in the form of these antibodies, which offer protection against future infections.

Vaccination: Anti-HBs can also develop as a result of the Hepatitis B vaccine. The goal of vaccination is to stimulate the immune system to produce these protective antibodies without causing the actual infection. A positive Anti-HBs test, in the absence of other Hepatitis B markers, usually indicates immunity from successful vaccination.

It’s important to note that while the presence of Anti-HBs typically suggests immunity, their absence doesn’t necessarily mean a person is susceptible to Hepatitis B. For example, some individuals who have recovered from Hepatitis B may have Anti-HBs levels that are too low to detect but are still immune. In cases of uncertainty, further testing is recommended.

Hepatitis C

Hepatitis C is a blood-borne virus that primarily affects the liver. In a home blood test for Hepatitis C, the key biomarker typically measured is the Hepatitis C antibody (Anti-HCV).

Anti-HCV antibodies are produced by the immune system in response to the Hepatitis C virus. When the body encounters the virus, it produces these antibodies to fight the infection. Therefore, the presence of Anti-HCV in the blood generally indicates prior exposure to the Hepatitis C virus.

However, there are a few important considerations regarding this biomarker:

Does not distinguish between past and current infection: A positive Anti-HCV result does not differentiate between a past infection, where the body has cleared the virus, and an ongoing or chronic infection. Both scenarios will show the presence of Anti-HCV antibodies, so the test alone cannot confirm whether the infection is active.

Does not indicate immunity: Unlike some other infections, clearing Hepatitis C or being successfully treated for it does not provide immunity against future infections. A positive Anti-HCV result does not imply protection from future exposure to the virus.

Given these limitations, a positive Anti-HCV result from a home blood test should be followed up with further testing, such as an RNA test, in a healthcare setting. This additional test can confirm whether the virus is still present in the bloodstream and help determine the next steps for treatment or management.

Who is at risk of Blood-Borne Viruses (BBVs)?

Blood-borne viruses (BBVs) such as Hepatitis B, Hepatitis C, and HIV can affect anyone, but certain groups are at higher risk due to exposure to specific risk factors. Below are some groups that are typically at increased risk of contracting a BBV:

People who inject drugs: Sharing needles, syringes, or other drug equipment can transmit BBVs.

Healthcare workers: Accidental needlestick injuries or exposure to patient bodily fluids can put healthcare workers at risk.

People with multiple sexual partners or unprotected sex: BBVs can be transmitted through sexual contact, particularly when protection is not used.

People who have received blood transfusions or organ transplants: This is particularly a risk for those who underwent these procedures before blood screening protocols were strict.

People with tattoos or body piercings: If equipment used is not sterile, BBVs can be transmitted.

People who travel or work in regions with high BBV prevalence: Certain areas have higher rates of BBVs, and exposure can increase risk.

Individuals born to mothers with a BBV: Vertical transmission can occur during childbirth if the mother is infected.

Men who have sex with men (MSM): This group faces a higher risk of HIV and Hepatitis B and C transmission.

People in correctional facilities: Overcrowded conditions and risky behaviors, such as sharing needles or having unprotected sex, increase the likelihood of BBV transmission.

People on hemodialysis: The blood circulation process in hemodialysis can pose a risk if strict infection control measures are not followed.

Preventative measures, such as vaccination (for Hepatitis B), practicing safe sex, using sterile needles (for both medical procedures and drug use), regular screenings, and seeking early treatment, can significantly reduce the risk of acquiring or spreading these viruses.
